. As TMZ first reported, Octo filed for Chapter 7 on April 30 ... the most serious form of bankruptcy. But according to docs filed today, Nadya didn't file all the necessary paperwork in time -- and never asked the court for an extension -- so her bankruptcy case was dismissed.
